My immediate reaction to this is to ask if the hardware is the same on each of the servers?
The reason for this is that I had an issue trying to do an upgrade from 4.1 to 5 and it came down to the hardware; as a result, I couldn't use the VMware issued upgrade, but had to get a modified version from the hardware vendor. It was something to do with the driver for the RAID card which was not supported by the standard VMware upgrade.
